In order to make sure your visit to the practice goes smoothly, we've put together all the important information about how things work at the practice.

Insurance

I do not bill insurance companies myself, but will give you a statement that you can submit to your insurance company for reimbursement.  Please click the Insurance tab on the left for further information.

Cancellation Policy

I have a 48 hour cancellation policy, not including weekends or holidays.  If you cancel sooner than 48 hours, you will be charged a regular fee unless I am able to fill that slot.  Insurance companies will not reimburse you for late cancellations or no-shows.
